# Capturing Source Format Metadata

You can set a [form](/v1/docs/creating-a-form)’s *Scope*setting to **Source Metadata**to associate the form submission with a [source format](/v1/docs/source-formats-overview). This form can determine return on investment for name buys, tracking costs of search lists, etc.

This form can collect information related to the source format or not.

## Creating source-scoped fields

To create a source-specific field:

1. Go to **Database → Fields**.
2. Select **New Field.**
3. [Configure the field settings as necessary](/v1/docs/fields-settings).
4. Set the **Scope**to **Source**.
5. Select **Save**.

Repeat as necessary.

## Creating source metadata-scoped forms

To create a Source Metadata scoped form:

1. Go to **Forms.**
2. Select **New Form**.
3. Give the form a name and set the status to **Active/Confirmed**.
4. Remove all fields from the form.
5. Select **Edit Properties.**
6. Set the **Scope**to **Source Metadata.**
7. [Add form fields from the palette](/v1/docs/form-palette).
8. [Map the form fields](/v1/docs/adding-standard-slate-fields) to the newly created Source-scoped fields.

## Associating the form with a source format

To associate a Source Metadata scoped form with a source format:

1. Go to **Database** → **Source Formats**.
2. On the source format with which the source metadata form should be associated, select **Edit**.
3. Select the desired form for the **Source Metadata** setting.

## Interacting with the form & viewing the data

You can find your new form in **Database → Upload Dataset**.

Select the source upload and then select **Metadata**.

On the form overview page, you can select **New Query**to export all Source metadata values.
