# Contact Information

Forms can collect contact information for follow-up and outreach. Email address and mailing address fields are included by default on many forms. If the form should also collect a phone number, add a Text Box field and map it to the appropriate phone system field.

Use the form palette to add a Text Box to a form. Configure the following settings:

- **Status:** Active
- **Type:** Select the appropriate field type for the question.
- **Label:** Enter the question text.
- **System Field:** Map this field to the appropriate system field. The first menu corresponds with the different sections of the person record under the **Profile** tab. After a section is selected, choose the corresponding system field from the list. For example, phone number fields are in the **Record** section of the person record.
- **Export Key:** The export key populates automatically based on the selected system field. For example, a phone field can use `sys:phone`.
- **Data Type:** Change the data type to **Telephone**. This automatically formats valid phone numbers entered on the form. When invalid characters, such as letters, are entered, the value displays in red to indicate an entry error.
- **Size:** Change the size to 32 so this field matches the other field widths.
- **Options:** Select **Required** if the field should be required. The form cannot be submitted without responses for required fields.

After a phone number is entered, the Phone Number field validates and formats the submitted information.
