# Custom Unsubscribe Follow-Up Form

Deliver uses [opt-out message groups](/v1/docs/opt-out-of-specific-communications-with-message-groups)to let users to unsubscribe (or opt out) from the messages you assign to those groups.

You can configure message groups to redirect unsubscribing users to a form that collects feedback about their decision to unsubscribe.

To do this, we’ll:

1. Create a landing form with follow-up prompts.
2. Create the custom message group, directing users to the landing form when they unsubscribe.
3. Create the mailing, specifying the custom message group for unsubscribing (opt-outs).

## Step 1: Creating the landing (follow-up) form

1. Click **Forms** on the top navigation bar.
2. Click **New Form**. The **New Form** popup appears.
3. Provide the required configurations.
4. Click **Save**. The form summary page appears.
5. Click **Edit Form**.
6. Delete all of the default form elements. Use `CTRL+click` to select several forms and delete them as a group.
7. Add the desired new form elements (for example, survey questions or feedback fields) to meet your data collection requirements.
8. Click **Edit Properties**. The **Edit Properties** popup appears.
9. For **Scope**, select **Unsubscribe Survey**.
10. Click **Save**.

## Step 2: Creating the Message Group

1. Click **Database** on the top navigation bar.
2. In the **Communications** section, select **Message Groups**. The **Message Groups** page appears.
3. Click the **Insert** link. The **Insert Record in Message Group** popup appears.
4. Set the configurations.
  - **Status**: Set to **Active**.
  - **Folder**: If using folder organization (recommended), select an existing folder or select **Other** and provide a new folder name.
  - **Name**: Give the message group an appropriate name. This only appears administratively.
  - **Unsubscribe Organization**: Optionally give the message group an organization. This organization will appear in the message footer above the unsubscribe link.
  - **Unsubscribe Group**: Give the group a descriptive name. This name will appear in the message footer and is the external name for the message group.
  - **Unsubscribe Message**: Use the WYSIWYG editor to craft an appropriate message for the user who is unsubscribing.
  - **Unsubscribe Form**: Select the form created in the previous section of this article.
5. Click **Save**.

## Step 3: Creating the Deliver Message

1. Click **Deliver** on the top navigation bar.
2. Click **New Mailing**.
3. Create the mailing as usual.
4. For **Opt Out**, select the Message Group created in the previous section of this article.

When the message recipient unsubscribes (opts out), they are directed to the custom landing page for the message group, and they will be offered the form to provide follow-up information.
