# Decision Reply Form

Accepted and waitlisted applicants can respond to your [decision letters](/v1/docs/decision-letters-decision-letters) using a **reply form**. We’ll start by making the form that admitted students see once they view the admission letter on the [Applicant Status page](/v1/docs/custom-applicant-status-portal).

## Creating a reply form

Make this an easy form to complete: the only data that absolutely needs to be captured on the form is the applicant's accepting your offer of admission. Encourage submission by limiting the number of additional questions you ask. For example, a common form might ask just one additional question for applicants who are declining the offer of admission: *Which institution will you attend instead?*

> Do not use the [Edit URL](/v1/docs/creating-a-form#manage-the-form) feature for reply forms, or for forms added as a checklist item. Using a simplified URL disables checklist-specific functionality, including ability to edit after submission, and expiration.

1. Go to **Forms.**
2. Select **New Form.**
3. Configure the following settings:
  - **Page Title**: Give your form a clear name.
  - **Folder**: Create a new folder called `Reply Forms`.
  - **Status**: Confirmed/Active
  - **Deadline**: Do not set a deadline. You may set a deadline for applicants to respond when you create rules for the reply form.
  - **Timezone**: Leave blank.
  - **Registrant Limit**: Leave blank.
  - **Privacy**: Confirm this is not selected.
  - **User**: Optionally associate the form with a Slate user.
4. Select **Save.**

## Configuring form properties

1. Select **Edit Form.**
2. Select **Edit Properties.**
3. Configure the following settings:
  - **Scope:**Application
  - **Multiple:**Confirm this is not selected.
  - **Require Checklist:**Select this option.
4. Select **Save.**

## Adding form content

Use the [form palette](/v1/docs/form-palette) to construct the reply form following these guidelines:

#### Don’t ask for personal information like name, address, or email

Applicants can only access this form by logging in to their applicant status page. Slate already knows which record is associated with the form response. Including personal information fields could allow applicants to change information that could cause data problems (for example, if a parent updates the name on a form to their own name).

#### You must include one required field

*Do you accept the offer of admission?*is the most important question on your form. The applicant’s response will initiate your [post-decisions rules](/v1/docs/decisions-automations).

Provide only two clear possible answers for the reply question:

- `I ACCEPT my offer of admission.`
- `I DECLINE my offer of admission.`

Use [option buttons](/v1/docs/form-palette#option-buttons) so the applicant can select only one response.
