If your process calls for the collection and review of data pertaining to activities such as events and travel, you may consider using a Form/Events Notes scoped form. This article will provide a step-by-step process on how to create and assign notes to events, forms, and trips within Slate.

Creating a notes form

  1. Select Forms in the top navigation bar.

  2. Select New Form

  3. Enter the following User configurations in the popup window:

    • Page Title - The Page Title will display to the user as the title of your form.

    • Folder - Keep your forms organized by putting your form in a folder. Select Other to create a new folder.

    • Status - Set the Status to Confirmed/Active.

  4. Select Save.

  5. Select Edit Form.

  6. Select Edit Properties and set the scope to Form/Events Notes.

  7. Select Save.

Assigning the notes form to an event, event template, or form

  1. Select Forms or Events in the top navigation bar.

  2. Select the form, event, or event template to which the Notes form should be assigned.

  3. Select Edit Form.

  4. Select Edit Properties and select the Form/Event Notes-scoped form from the Notes Form select list.

  5. Select Save.

  6. Navigate back to the form, event, or event template that the form was assigned to and select Notes.

  7. The Notes form will now display when selecting Notes in the top right corner.

Assign the form to a trip

  1. Select Events in the top navigation bar.

  2. Select on Trips in the Events drop-down menu.

  3. Select on the trip you wish to edit.

  4. Select Edit and select the Form/Event Notes scoped-form via the Notes Form drop-down.

  5. Select Save.

  6. Navigate back to the trip that the form was assigned to and select Notes.

  7. The Notes form will now display when selecting Notes.
