Video Capture Form Widget

The video capture form widget, located in the form palette, lets you collect recorded video from a form registrant on various scoped forms including person and application scoped.
Adding a video capture widget to a form
To add a video capture widget to a form:
Go to Forms.
Select New Form.
Configure the form details, then select Save.
Select Edit Form.
From the palette, select Video Capture.
Configure the following settings:
Status: Select Active or Inactive.
Label: Add an optional text label to the widget. Select Hide Label to hide it from registrants.
Script Key: Enter a unique key to be referenced by a calculated field on the form. For an example of a script key in action, see Translation Codes.
Column Width: Specify column width in percent. Leave blank for a single column layout.
Layout: Select from one of three options:
Default: Respects the form’s layout setting in Edit Properties → Layout.
Stacked: Overrides the default setting to place the text label above the widget’s content.
Table: Overrides the default setting to place the text label to the left of the widget’s content.
Options: Select one or both:
Internal Only: The widget appears to administrative users only.
Do not display after submission: The widget will not be available to registrants after they submit the form.
Question/Prompt: Enter a prompt to guide the registrant’s recording. This prompt appears only after the registrant selects Ready.
Prep Time: Enter an amount of time in seconds. This time counts down when the registrant selects Ready, then recording begins.
Recording Time: The length in seconds that registrants are permitted to record their video response.
Format: Select one of two formats:
Video Essay: The default selection. Records a video of a given length.
AI Interview: Prompt Slate AI to conduct a video interview. See AI Interviews for more information.
(🏔️ Summit 2025 Feature) Maximum Attempts: Video essay only. Enter the number of times a registrant may re-record their video.
Select Save.
Helpful Tips for Setting up Attempts:
Clicking “Ready” on the widget starts the first attempt.
Clicking “Restart” begins the second or next attempt depending on how many are configured.
The “Restart” button only appears if more than one attempt is configured in the widget settings.
If only one attempt is allowed, consider adding clear instructional text to let applicants know they won’t be able to try again if they click “Stop” before they finish recording.
If multiple attempts are allowed and the first attempt is completed (e.g., by clicking “Stop” or using the full time), clicking “Restart” will begin the next available attempt.
Helpful Tips for Testing:
For the most accurate representation of what applicants will experience, we recommend testing the form using the Form URL rather than using the admin New Registration button.
If you have an application-scoped form, you can replicate the applicant experience by impersonating an application record and then accessing the form via that same Form URL.
Testing this feature as an admin or applicant will count all the attempts, so using a new record each time will be more accurate.
